Student Information Form

(Refer to Board Policy JBC and JRA) 

Each year during enrollment, parents, and students must fill out an information form requesting: ● Name(s), phone number(s) and address of the student’s parent(s)/guardian(s); 

● Name and phone number of student’s emergency contact(s); 

● Name of the student’s physician; 

● Description of any medical conditions of which the staff needs to be aware. 

  • Enrollment/Withdrawal from School
